Since founding Partners in Care in 1997, June has pioneered the development and scaling of evidence-based innovative interventions for management of medications at home, self-management of chronic conditions, coordinated care to improve health outcomes, and care transitions.
Win Whitcomb, MD, MHM, is Chief Medical Officer of Remedy Partners, where he oversees all aspects of the clinical enterprise, and the intersection between clinical, technology, and business. Prior to joining Remedy, Dr. Whitcomb led the development and implementation of several bundled payment programs at Baystate Health in conjunction with its commercial health plan and the Center for Medicare and Medicaid Innovation.
Dr. Manoj K. Mathew is a physician executive with valuable experience and insight in population health and coordinated care. As an Internal Medicine physician with Board Certification in Hospice and Palliative Care, he began his career in 2000 as a PCP and then focused on Acute and Post-Acute care.
